This is the Atlantis Models "1/150 Convair Space Shuttlecraft (formerly Revell)" plastic model.

This winged shuttle and rocket booster was presented as a concept model by Dr. Arnold Ehrlich Kraft, who worked in the Convair division of General Dynamics in the 1960s.
